This unique position’s primary function is to fulfill Catholic Charities commitment to serve individuals and families in an inclusive and holistic manner. An integral part of the Case Management team, this position will be based in the Yonkers and will also need to travel to the Brewster site as needed. The position provides direct assistance for those in need through case assistance and case management services.
Essential Duties and Responsibilities include the following:
Provides in-person Case Management to a broad population of individuals and families including but not limited to immigrants, senior citizens, people who are homeless or facing homelessnessetc.
Provides effective engagement, assessment, service planning and follow up using a strength’s based and client-centeredapproach.
Provides crisis intervention in emergencies around shelter, food, clothing, and other basic needsthrough the provision of financial assistance, goods, advocacy, benefits access and access to relevant resources.
Conducts thorough assessments beyond the presenting need and develops a service plan that addresses immediate need and plans towardself-sufficiency.
Develops a comprehensive understanding of the complex social service system and helps clients navigate these systemsaccordingly.
Conducts outreach in thecommunity
Assists in applying for public and privatebenefits
Provides Resource and benefits access through direct assistance and/or referral.
Developsknowledgeaboutandmaintainscollaborativerelationshipswithcommunityagencies,other Catholic Charities Departments, Divisions, Affiliates and Archdiocesanentities.
Develops a capacity to provide or ensure access to a wide range of social service benefits and helps develop and maintain a network of high-qualityresources.
Fulfills operational and otherduties.
Enters all information into the ECM database, meets program deliverables in a timely and efficientmanner, and understands the significance of documentation to funders and otherstakeholders.
Completes all requests for financial assistance and gathers relevant supportive documentation in a timely, efficient, and thoroughmanner.
Actively participates in weeklysupervision.
Attends and participates in monthly Department meetings, community meetings, trainings, and other agency-relatedactivities.
Assists at other sites asneeded.
Other duties asassigned.
Salary: $26.37 - $27.47 per hour, this is a non-exempt hourly position based on a 35 hour work week or $48,000 -$50,000 annually.
Position Type and Expected Hours of Work:
This is a full-time position. Days and hours of work are generally 9:00 AM to 5 PM Monday - Friday. Additional hours may be required to meet program deadlines, or client needs. There may be periods when work hours vary.

Travel to various pantry sites, outreach, and staff meetings. The ability to travel using reliable transportation, traversing streets, ascending/descending stairs, and exposed to outside weather conditions.
Traverse neighborhoods that may require a high level of alertness and awareness ofsurroundings.
Work with clients who require consistent and comprehensive support, some under potentiallystressful conditions.
Remain in a stationary position at a workstation and use a computer approximately 60% of thetime.
Excellent interpersonal skills; the ability to work well independently and as part of ateam.
Knowledge of social service delivery systems and localresources.
Excellent organizational, time-management and communicationskills.
WIFI access and an ability to successfully navigate remotetechnology.
